All volunteers are asked to complete a volunteer disclosure on-line application.
Before you begin the process of applying, please notify your child’s school office personnel that you would like to volunteer. It is imperative that you communicate this to your child’s school so that your Background check can be completed. Failure to do this will result in an incomplete Background check.
Step 1 | Contact your students school office personnel
Step 2 | Complete online application
Step 3 | A building employee will contact volunteer with more information, when background check is cleared
The tremendous responsibility the Middleton-Cross Plains Area School District has to its school children and community necessitates the information from all volunteers regarding convictions.*
*A record of conviction does not prohibit placement; however, failure to complete the online disclosure application accurately and completely may mean disqualification from consideration for placement or may be cause for dismissal if placed. Applicants must report any convictions that occur subsequent to the time they initially completed the online disclosure application.
